# Oral-Only Interventions Across Every Layer of Skin Structure: A Complete Audit

## TL;DR
- **The oral skin-aging field has now expanded well beyond Type I/III collagen and HA: there are genuine human RCTs for oral elastin peptides (VGPG Elastin), eggshell membrane (MKARE), phytoceramides (Lipowheat, rice/konjac), MSM, hyaluronic acid, postbiotics, and choline-stabilized silicon (BioSil) — but most remaining targets (Type IV/V/VII collagen, decorin, copper, manganese, PEA, urolithin A, spermidine) have NO oral skin RCT and rest only on mechanism or topical/non-skin data.**
- **The single most important caveat across the entire field: Myung & Park (Am J Med 2025;138(9):1264–1277) — a meta-analysis of 23 RCTs (1,474 participants) — found "studies not receiving funding from pharmaceutical companies revealed no effect of collagen supplements for improving skin hydration, elasticity, and wrinkles, while those receiving funding from pharmaceutical companies did show significant effects," concluding there is "no clinical evidence to support the use of collagen supplements to prevent or treat skin aging." 21 of 23 trials had industry involvement. Apply this skepticism to nearly every positive trial below.**
- **Highest-value untapped oral targets with real (if preliminary) evidence: oral elastin peptide (100 mg/d), eggshell membrane (300 mg/d, which beat 8 g of collagen head-to-head on elasticity), phytoceramides (350 mg/d wheat or ~40 mg rice), and carnosine (anti-glycation). Urolithin A, spermidine, and PEA remain genuine oral-skin gaps despite strong adjacent data.**

## Key Findings

The oral nutricosmetic evidence base is deep for a narrow set of "structural substrate" ingredients (collagen peptides, hyaluronic acid, ceramides) and for broad antioxidant/photoprotection blends, but it is thin or absent for the specific molecular targets in this audit (basement-membrane collagens, individual proteoglycans, trace-mineral cofactors, and most emerging bioactives). Where oral RCTs are missing, the literature is dominated by (a) topical trials, (b) in-vitro/keratinocyte expression studies, or (c) general-health trials in other organs.

A recurring pattern: ingredients with the strongest mechanistic rationale for a specific skin layer (e.g., copper for lysyl-oxidase cross-linking; collagen VII for anchoring fibrils) have been developed almost exclusively as topical or device products, not oral supplements, because demonstrating systemic delivery to a specific dermal compartment is pharmacologically hard.

## Details by Category

### STRUCTURAL COMPONENTS

**1. Collagen types beyond I/III (IV, V, VII).**
- *Covered:* Type I/III via hydrolyzed peptides — the entire oral collagen evidence base is Type I/III substrate provision.
- *Missing:* No oral supplement is marketed or trialed to selectively raise Type IV (basement membrane), V (fibrillogenesis regulator), or VII (anchoring fibrils). The DEJ-protein work that exists is entirely **topical or in-vitro**: a peptide complex stimulated collagen XVII, laminin and nidogen in cultured keratinocytes and ex-vivo human skin (Jeong et al., *Int J Mol Sci* 2019;21:73), and topical palmitoyl-RGD raised laminin-332, collagen IV and collagen XVII dose-dependently in HaCaT keratinocytes (*Mol Med Rep* 2022).
- *Rationale:* Type VII anchoring fibrils and Type IV lamina densa decline with age, increasing DEJ flattening and skin fragility; reinforcing them would theoretically reduce wrinkling and shear fragility.
- *Evidence/dose/cost:* No oral RCT; no dose; n/a. **Genuine white space.**

**2. Elastin and elastin-associated proteins (fibrillin-1, fibulin, lysyl oxidase).**
- *Covered (broadly):* elastin synthesis. *NEW and important:* there IS now an oral elastin RCT. Seong SH et al. (*Skin Res Technol* 2024;30(3):e13634; doi:10.1111/srt.13634) randomized 100 Korean adults to **100 mg/day Bonito (Katsuwonus pelamis) fish-derived elastin peptide (VGPG Elastin®, Daehan Chemtech, Seoul; total average MW 582 Da)** vs placebo for 12 weeks; the active group showed significant improvement in skin roughness, multiple wrinkle parameters, eye-wrinkle volume and hydration, with a lower melanin index and no adverse events. Verbatim: "Oral consumption of Bonito elastin peptide (VGPG Elastin®) reduced fine wrinkles, enhanced skin moisture, and decreased melanin index without significant adverse effects." Mechanistically, elastin peptides (rich in Gly/Ala/Val/Pro, VGVAPG motifs) promote fibroblast proliferation and elastin synthesis.
- The Proksch 2014 Verisol trial also measured elastin: **+18% elastin (p<0.01)** and **fibrillin +6% (non-significant)** in suction-blister fluid.
- *Missing:* No oral intervention specifically targets lysyl oxidase (LOX) cross-linking activity, fibulin, or elastin fiber *quality/stabilization* (vs synthesis). LOX is copper-dependent (see copper below).
- *Dose/cost:* Elastin peptide ~100 mg/day; VGPG Elastin is a branded Korean ingredient, modest retail cost.

**3. Proteoglycans and GAGs (HA, chondroitin, keratan, decorin, versican, aggrecan).**
- *Covered:* Oral HA has robust RCT support — Hsu et al. (*Nutrients* 2021;13:2220): 120 mg/day HA for 12 weeks improved wrinkles, hydration, TEWL and elasticity in 40 adults; a 150-person RCT (*Sci Rep* 2025) of oral sodium hyaluronate improved hydration, barrier and aging signs; a 2025 meta-analysis of 7 RCTs found significant improvement in hydration, elasticity and wrinkle depth.
- *Newer:* A "hyaluronic acid matrix" (Dermial®, 60 mg/day) containing HA + sulfated GAGs + collagen improved brightness, hydration, smoothness and roughness (*Dermatol Ther* 2025).
- *Missing:* No standalone oral RCT for **chondroitin sulfate, keratan sulfate, decorin, versican, or aggrecan** as skin-aging interventions. Oral chondroitin RCTs exist only for osteoarthritis, usually combined with glucosamine/HA. Decorin (which regulates collagen fibril diameter) has no oral product.
- *Dose/cost:* HA 120–200 mg/day, inexpensive.

**4. Lipids and barrier lipids (ceramides, cholesterol, FFA, phospholipids, sphingosine, PEA).**
- *Covered:* Oral phytoceramides have multiple RCTs. Guillou et al. 2011: **350 mg/day wheat-derived phytoceramides** for 12 weeks significantly improved hydration and reduced TEWL; wheat polar lipid complex (Bizot/WPLC, *Cosmetics* 2017;4:37): 1.7 mg glucosylceramides + 11.5 mg DGDG for 60 days improved hydration, elasticity, smoothness, and reduced TEWL/roughness/wrinkles (all p<0.001); rice-derived glucosylceramides ~1.8 mg/day improved hydration ~32% (arm)/23% (cheek).
- *Missing:* **No oral RCT for cholesterol supplementation, specific phospholipid combinations (PC/PE/PS), saturated:unsaturated FA-ratio optimization, or sphingosine/sphinganine for skin.** Oral **PEA** has RCTs only in atopic dermatitis/eczema (largely in dogs, or as a topical in humans — e.g., the Levagen+ topical eczema RCT, *Skin Pharmacol Physiol* 2023); no oral PEA skin-aging trial.
- *Dose/cost:* Wheat phytoceramides 350 mg/day or rice ceramides 40 mg/day; cheap.

**5. Dermal-epidermal junction (DEJ) proteins.**
- *Covered:* nothing orally.
- *Missing:* All DEJ-reinforcement evidence (collagen VII/XVII, laminin-332, nidogen, integrins, hemidesmosomes) is **topical or in-vitro** (Jeong 2019; palmitoyl-RGD 2022). No oral intervention demonstrates DEJ reinforcement. White space.

**6. Fibroblasts and fibroblast function.**
- *Covered (indirectly):* collagen di/tripeptides Pro-Hyp and Hyp-Gly are absorbed intact and act as **signaling molecules** that stimulate fibroblast proliferation and migration on collagen gels (Shigemura et al.), not merely as amino-acid substrate — this is the mechanistic basis for collagen peptides being more than protein.
- *Missing:* No oral intervention specifically targets TGF-β signaling, growth-factor delivery, or fibroblast migration as a primary endpoint. Oral growth factors are not bioavailable. White space beyond peptide signaling.

**7. Keratinocytes and epidermal barrier (desmogleins, claudins, occludin, tight junctions).**
- *Covered:* Barrier function is measured (TEWL) in ceramide/HA/collagen trials, but no oral intervention targets tight-junction proteins (claudin/occludin) or desmogleins directly.
- *Missing:* White space; tight-junction modulation evidence is in-vitro only.

**8. MMPs and TIMPs.**
- *Covered:* Several trials report downstream MMP suppression — astaxanthin + collagen reduced MMP-1 and MMP-12 expression (Yoon 2014, *J Med Food*); collagen peptides discussed as decreasing MMP-2 (Zague 2011).
- *Missing:* **No oral compound is trialed specifically to upregulate TIMPs (tissue inhibitors of metalloproteinases).** TIMP enhancement is mechanistic/in-vitro only.

**9. Cross-linking, maturation, and glycation/AGE inhibitors.**
- *Covered:* Vitamin C + silicon for hydroxylation. Silicon: Barel A, Calomme M, Timchenko A et al. (*Arch Dermatol Res* 2005;297(4):147–153; doi:10.1007/s00403-005-0584-6) gave 50 women with photodamaged skin 10 mg Si/day as choline-stabilized orthosilicic acid (ch-OSA/BioSil; n=25) vs placebo (n=25) for 20 weeks: "Oral intake of ch-OSA during the 20 weeks results in a significant positive effect on skin surface and skin mechanical properties, and on brittleness of hair and nails" — a real RCT (funded by manufacturer Bio Minerals n.v.).
- *Newer (AGE/glycation):* **Carnosine** is the key anti-glycation oral candidate — an oral non-hydrolyzed carnosine/carcinine formulation (Can-C Plus) showed continuous significant improvement in skin parameters and fine lines over 3 months vs placebo; carnosine scavenges carbonyls and may disrupt AGE cross-links (*Rejuvenation Res* 2011). A multi-ingredient capsule (vitamins + amino acids + carnosine) improved skin surface parameters vs placebo in diabetics. The AGE cross-link breaker **ALT-711 (alagebrium)** is investigational and not an oral skin product.
- *Missing:* No oral intervention for aldol cross-links specifically; AGE-breaker drugs not commercialized for skin.
- *Dose/cost:* Carnosine commonly 500 mg/day; silicon 10 mg/day; both inexpensive.

**10. Microcirculation and perfusion.**
- *Covered:* cocoa flavanols.
- *Newer:* **Dietary nitrate / beetroot juice** robustly raises NO and improves microvascular blood flow (Xtreme Alps RCT at altitude; multiple cardiovascular RCTs), but **no RCT measures dermal/facial skin microcirculation or skin-aging endpoints** — the perfusion benefit is demonstrated in muscle, sublingual and forearm vasculature. L-arginine and L-citrulline are NO precursors with wound-healing (not skin-aging) data.
- *Missing:* A dedicated oral nitrate/beetroot skin-aging RCT. White space with strong rationale.

### MISSING AMINO ACIDS & PEPTIDES

**Individual amino acids.** L-arginine has the best evidence but for **wound healing, not aging**: oral arginine enhanced collagen deposition (hydroxyproline) and wound breaking strength in humans (Barbul/Kirk, *Surgery* 1990) and in combined arginine + zinc + vitamin C surgical collagen-synthesis trials (e.g., NCT03221686: 15 g arginine + 55 mg zinc + 1,251 mg vitamin C). Glycine and proline are studied only as collagen substrate, not as standalone skin-aging RCTs. **No standalone oral skin-aging RCTs exist for L-proline, L-lysine, L-glycine, L-taurine, L-carnitine, or L-citrulline.** L-carnosine appears in combination skin formulas (with HA and MSM — an HA + L-carnosine + MSM nutraceutical improved glabella elasticity +22.6% and hydration in an RCT, *J Clin Aesthet Dermatol* 2019).

**Specific dipeptides.** The key comparative finding: Inoue N, Sugihara F, Wang X (*J Sci Food Agric* 2016;96:4077–4081) compared a collagen hydrolysate **high in Pro-Hyp + Hyp-Gly vs low**, both 5 g/day, in 85 women — the high-dipeptide product produced significantly more improvement in moisture, elasticity, wrinkles and roughness (industry-funded, Nitta Gelatin). No RCT compares Ala-Gly, Gly-Gly, or Gly-Ala head-to-head. Pro-Hyp and Hyp-Gly are the dominant bioactive plasma peptides; plasma Hyp-peptides rise dose-dependently (≈6.4 → 32.8 nmol/mL across rising hydrolysate doses).

**Bioactive peptides from non-collagen sources.** *NEW:* **Eggshell membrane (MKARE®)** — González-Rodríguez Y et al. (*Front Nutr* 2026;12:1689701; doi:10.3389/fnut.2025.1689701) randomized 62 women (40–70) to 300 mg/day eggshell membrane vs **8,000 mg/day hydrolyzed collagen** vs placebo over 8 weeks; eggshell membrane significantly improved firmness/flexibility and elasticity at day 57 — concluding the "300 mg intake of fresh membrane MKARE positively affects mechanical skin parameters compared to the group that consumed 8,000 mg of hydrolyzed collagen or the placebo group" (R0 +44.7%, R2 ~+40%; funded by ingredient maker Arandovo SL). Whey/casein/wheat/soy peptides have general-health data but no dedicated skin-aging RCT (wheat *oil* extract + HA has one — Vance et al., *Dermatol Ther* 2025, reduced crow's feet).

**Amino-acid ratios for fibroblast stimulation.** Animal wound data show arginine + proline synergy (USPTO enteral-formula data); no human skin-aging RCT defines an optimal ratio.

---

### OVERLOOKED COFACTORS & MICRONUTRIENTS

- **Copper (lysyl-oxidase cofactor):** No oral copper RCT for skin aging. Evidence is topical (copper-peptide GHK-Cu serums; copper-oxide pillowcases reduced wrinkle depth — Borkow, *J Cosmet Dermatol* 2012). White space despite the strongest cross-linking rationale.
- **Zinc:** No standalone oral zinc skin-aging RCT; appears only in blends (marine complex + vitamin C + zinc, Imedeen-type; arginine + zinc + vitamin C wound trials).
- **Iron, Manganese:** No skin-aging RCT for either.
- **Sulfur (MSM, methionine/cysteine):** **MSM has real RCTs** — Anthonavage M, Benjamin R, Withee E (*Natural Medicine Journal* 2015;7(11)), N=20 women on OptiMSM 3 g/day for 16 weeks: "statistically significant improvement from mean baseline at week 8 (−0.31 cm ± 0.39, P=0.024) and week 16 (−0.84 cm ± 0.67, P=0.002) for crow's feet lines/wrinkles," plus elasticity (P=0.007) and firmness (P=0.004) at week 16. A dose-response follow-up (*Int J Vitam Nutr Res* 2020, doi:10.1024/0300-9831/a000643; N=63) found 1 g and 3 g/day both effective. Dose 1–3 g/day; cheap.
- **Choline and betaine:** No skin-aging RCT (choline appears only as the silicon stabilizer in ch-OSA).
- **B vitamins (B6, B12, folate):** No dedicated skin-aging RCT; only in multivitamin blends.

---

### EMERGING BIOACTIVES

- **PEA:** Oral RCTs in atopic dermatitis (dogs) and topical eczema (humans); **no oral skin-aging RCT.**
- **Urolithin A:** Key correction — the skin RCTs (Mitopure/Timeline, *medRxiv* 2023) are **TOPICAL**, not oral. Oral urolithin A RCTs exist for muscle and immune aging (1,000 mg/day; *Nature Aging* 2025, NCT05735886) but **none for skin.** Strong rationale (mitophagy; collagen upregulation in vitro) but oral skin data absent.
- **Spermidine:** Oral RCT exists for hair (anagen prolongation, *Dermatol Pract Concept* 2017) and cognition (*JAMA Netw Open* 2022); skin claims rest on a small Kim 2021 study and topical/in-vitro work. A pharmacokinetic RCT found oral spermidine (15 mg/d) raised plasma spermine but not spermidine. **No robust oral spermidine skin-aging RCT.**
- **Postbiotics/SCFA (gut-skin axis):** *NEW and strong:* heat-killed *Lactobacillus acidophilus* IDCC 3302 (*Nutrients* 2025;18:596) — 100 participants, 12 weeks, primary endpoint crow's-feet wrinkles; postbiotic VMK223 (500 mg/day; *Dermatol Ther* 2026) improved wrinkle depth, hydration and elasticity in 34 women; KABP-065 postbiotic improved dryness, elasticity and TEWL. No isolated SCFA (butyrate) skin RCT yet.
- **Collagen metabolites:** The emerging understanding is that Pro-Hyp/Hyp-Gly act as signaling peptides (above) — the "metabolite beyond amino acids" story.
- **Exosomal/microvesicle oral supplements:** No oral exosome skin RCT; exosome skin evidence is injectable/topical only. White space (and a regulatory grey zone).

---

### COLLAGEN QUALITY & SOURCING

- **Molecular-weight head-to-head:** No clean RCT isolates MW alone. Best comparative data = Inoue 2016 (high vs low dipeptide content). Pharmacokinetics support low-MW: a hydrolysate with 89% of components <3,000 Da raised plasma Pro-Hyp/Hyp-Gly more than 100,000-Da gelatin (free Hyp 169 vs 94 nmol/mL). Counterpoint: Chai 2010 found heavier fish-scale peptides (3,500–4,500 Da) penetrated *better transdermally* — complicating "smaller is always better."
- **Sourcing (bovine/marine/porcine/plant):** No head-to-head RCT for skin outcomes; "neither source has demonstrated clear clinical superiority for skin outcomes in head-to-head trials." Plant "collagen" does not exist (it is amino-acid/cofactor blends). Marine is ~Type I only; bovine adds Type III and has the largest evidence base; Proksch's Verisol is porcine. A randomized crossover bioavailability RCT (Virgilio et al., *Front Nutr* 2024) found fish CH gave higher Hyp-Gly Cmax than bovine-LMW (p=0.011) and porcine (p=0.041) — absorption, not a skin endpoint.
- **Hydrolysis method:** Enzymatic hydrolysis generates the bioactive di/tripeptides; no RCT compares enzymatic vs acid vs alkaline for skin.
- **Purity/contamination:** The Clean Label Project 2020 white paper tested ~30 collagen products: 64% had measurable arsenic, 37% lead, 17% cadmium; ~13% failed maximum-limit standards (one Bulletproof Chocolate product exceeded both cadmium [9.17 µg/serving vs 4.1 limit] and lead [1.57 vs 0.5 µg/serving]). **Important: this is a non-peer-reviewed white paper, disputed by industry (Natural Products Association, CRN); "measurable" ≠ "exceeds safety limit," and most products passed.** ConsumerLab runs independent ICP-MS testing against the Prop 65 0.5 µg/serving lead limit. Choose NSF Certified for Sport / USP / Informed Sport products.
- **Collagen + cofactor combos:** No RCT compares collagen + cofactors vs collagen alone. Combination-vs-placebo RCTs exist — Czajka A et al. (*Nutr Res* 2018;57:97–108): 120 subjects, fish collagen + vitamins/antioxidants, 90 days, improved elasticity and dermal thickness and reduced photodamage inflammation; and a Verisol 2.5 g + HA 120 mg + vitamins A/C/D/E + zinc + selenium product improved wrinkles and elasticity. Synergy is plausible (vitamin C is a prolyl/lysyl-hydroxylase cofactor) but unproven head-to-head.

**The landmark Proksch trials (for reference):** Proksch E et al. (*Skin Pharmacol Physiol* 2014;27(3):113–119; doi:10.1159/000355523) — 114 women aged 45–65, **2.5 g/day Verisol (porcine type I, ~2.0 kDa)** for 8 weeks: eye-wrinkle volume **−20.1% at 8 weeks (p<0.01)**, **procollagen type I +65% (p<0.01)**, **elastin +18% (p<0.01)**, **fibrillin +6% (NS)**; MMP discussed (citing MMP-2 suppression from Zague 2011) but not assayed in this trial. The companion paper (27(1):47–55, n=69) compared **2.5 g vs 5 g**; both improved elasticity vs placebo.

---

### SKIN BARRIER & LIPID LAYER (under-researched)
- Oral cholesterol: no RCT. Phospholipid combinations (PC/PE/PS): no skin RCT. Saturated:unsaturated ratio: no skin RCT. Sphingosine/sphinganine: no oral RCT (only as ceramide precursors within phytoceramide products). The only well-evidenced oral barrier-lipid interventions remain phytoceramides (above) plus omega-3.

### DERMAL PERFUSION & OXYGENATION
- NO production: dietary nitrate/beetroot (strong vascular RCTs, no skin endpoint); L-arginine/L-citrulline (wound healing). Blood rheology / oxygen delivery to dermis: no dedicated oral skin RCT. Dermal anti-inflammation: covered indirectly by omega-3, polyphenols (trans-resveratrol RCT — *J Cosmet Dermatol* 2022, oral + topical), and postbiotics. Cocoa flavanols remain the best-evidenced oral perfusion intervention.

## Recommendations

**Stage 1 — Add the ingredients that now have real oral skin RCTs (deploy now):**
1. **Oral elastin peptide (VGPG Elastin), ~100 mg/day** — fills the elastin-fiber gap with a positive 100-person RCT.
2. **Eggshell membrane (MKARE), 300 mg/day** — beat 8 g of collagen head-to-head on elasticity; provides collagen + elastin + HA matrix peptides at low dose.
3. **Phytoceramides, 350 mg/day wheat (or 40 mg rice/konjac for wheat-sensitive)** — best-evidenced oral barrier-lipid intervention.
4. **MSM, 1–3 g/day** and **choline-stabilized silicon (ch-OSA/BioSil), 10 mg Si/day** — both have positive wrinkle/elasticity RCTs and cover the sulfur and cross-linking-cofactor gaps cheaply.
5. **Carnosine, 500 mg/day** — the practical oral anti-glycation/AGE option.
6. **A postbiotic** (e.g., heat-killed *L. acidophilus* IDCC 3302 or equivalent) for the gut-skin axis.

**Stage 2 — Track these emerging oral candidates; deploy when an oral skin RCT appears:**
- **Oral urolithin A** (currently topical-only for skin; oral data are muscle/immune) — watch for a skin-endpoint oral trial.
- **Dietary nitrate/beetroot** for dermal perfusion — strong vascular rationale, needs a skin-microcirculation RCT.
- **Spermidine** — needs a robust oral skin RCT; current skin data weak.
- **PEA** — needs an oral skin-aging (vs eczema) trial.

**Stage 3 — Genuine white spaces (no oral product/RCT; do not over-claim):** Type IV/V/VII collagen, DEJ reinforcement, decorin/keratan/chondroitin for skin, copper/zinc/iron/manganese standalone, TIMP upregulation, oral exosomes, oral cholesterol/phospholipids/sphingosine. Treat any product claiming these as mechanism-only.

**Benchmarks that would change the recommendation:** (a) a non-industry-funded, high-quality RCT confirming any collagen/HA/ceramide effect would upgrade confidence materially; (b) an oral urolithin A or beetroot trial with a skin-aging endpoint; (c) a clean MW-isolating collagen RCT; (d) a collagen-vs-collagen+cofactor head-to-head.

## Caveats
- **Industry funding dominates.** Myung & Park (*Am J Med* 2025;138(9):1264–1277; doi:10.1016/j.amjmed.2025.04.034), a meta-analysis of 23 RCTs (1,474 participants), found "studies not receiving funding from pharmaceutical companies revealed no effect of collagen supplements for improving skin hydration, elasticity, and wrinkles, while those receiving funding from pharmaceutical companies did show significant effects," concluding "no clinical evidence to support the use of collagen supplements to prevent or treat skin aging." 21 of 23 trials had industry involvement. The same bias pattern applies to most non-collagen ingredients above (elastin peptide, eggshell membrane, MSM, silicon, postbiotics are nearly all maker-funded).
- Many "skin" trials for promising molecules (urolithin A, PEA, RGD/DEJ peptides) are **topical or in-vitro**, not oral — do not conflate routes.
- Effect sizes for the genuinely positive oral RCTs are modest; endpoints are often cutometer/self-report rather than histology; sample sizes are small; durations short (6–16 weeks); and populations skew Asian/female.
- "Plant collagen" is a misnomer; oral exosome supplements are largely unvalidated and a regulatory grey area.
- Heavy-metal contamination data are real but the headline figures come from a disputed non-peer-reviewed white paper; use third-party-certified products.
