Sensory Syncrasis Evidence Base: Screens (Paper A) and ADHD (Paper B)

Summary
  • Paper A (Screens): Change is real but mostly domain-specific and often enhancing. Action video games causally improve perceptual and attentional skills (Bediou et al. 2018: g = 0.34 in intervention studies, g = 0.55 cross-sectionally). Gamers show tighter audiovisual temporal windows (Donohue et al. 2010). VR transiently increases depersonalization/derealization more than PC gaming does. Developmental "harm" findings come from observational cohorts (Law et al. 2023; Huang et al. 2026; Heffler et al. 2024) and cannot exclude reverse causation. Population-level associations with well-being are tiny (≤0.4% of variance; Orben & Przybylski 2019).
  • Paper B (ADHD): Rate the "different starting syncrasis" thesis partially supported. Interoceptive accuracy is lower in 3 of 5 case-control studies (Bruton et al. 2025), and a 2025 study (30 ADHD vs 57 controls) held up after confounds were controlled. However, a large 2026 psychophysics study found objective interoception largely unrelated to psychiatric symptoms. Gain/arousal accounts are the best grounded. Narrower (not wider) audiovisual binding windows were found in high-ADHD-trait adults.
  • Both papers: The single strongest design in Paper A is the outdoor-time myopia RCT (He et al. 2015: 30.4% vs 39.5% 3-year incidence), but it concerns near-work and light, not screens per se. Alderson-Day & Fernyhough (2015) noted that no studies had specifically examined inner speech in ADHD, and no ADHD affect-labeling study was found.

The evidence base can support only part of each paper. For screens, the strongest mechanism-level evidence shows the configuration changing. Action games sharpen visual and audiovisual temporal precision. VR transiently shifts the inside–outside balance. Outdoor time causally changes the eye's development. Claims that everyday screen use durably resets a child's "sense ratio," gain or interoception currently rest on small, mostly parent-report cohort studies with small effects. For ADHD, an altered starting configuration is partially supported on gain/arousal and interoception, but the interoception data are small, mixed and mostly use the heartbeat-counting task, whose validity is disputed. The integration-mode and inner-speech dimensions are largely speculative.

1Part A verdict

Screens change syncrasis in documented ways, but the best-designed evidence (RCTs, training studies, lab experiments) mostly shows short-term, context-bound shifts and skill-specific enhancements, not global degradation. The long-term developmental "persistence" claims are the paper's weakest link. The "mere presence" arousal story ("brain drain", Ward et al. 2017) has largely collapsed: the pre-registered replication and the largest meta-analysis (d = −0.02) failed to support it. Sense ratio is the most theoretically central dimension and the least studied: no one has measured Colavita dominance or Ernst–Banks cue weights as a function of media diet. That is a genuine gap and an opportunity.

2Part B verdicts by dimension

DimensionVerdict
Interoception (inside–outside)Partially supported (mixed; method-confounded)
Multisensory integration / sensory processingPartially supported for questionnaire-based sensory atypicality; not supported for "wider binding"
Gain / arousalSupported (strongest; mechanistic + modest behavioural) — white/pink noise g = 0.249 for ADHD groups (Nigg et al. 2024)
Predictive processing / computationalPartially supported (stochastic choice robust; learning-rate/volatility speculative)
Inner speech as regulatory mediumSpeculative (theory + childhood data; adult data contradict deficit framing)
Alexithymia / affect labelingPartially supported (alexithymia elevated); affect labeling untested
Screens as external regulationSpeculative (association modest, OR 1.10, Ra et al. 2018; mechanism unstudied)

Inner speech may be over- rather than under-produced: children with ADHD produce more (often task-irrelevant) private speech, and high-trait adults report more inner speech (Benfield et al. 2025). The issue may be regulatory coupling, not quantity. [Synthesis]

3Caveats

  • Not located in this search: Ren et al. 2024 NeuroImage, Vozel 2024 Children, Children 2024 11(4):464, the 2025 Pediatric Research toddler sensory-profile study, Ciccarelli et al. 2024. Check before citing.
  • Pickens et al. 2019 is a narrative review, not a meta-analysis; the meta-analysis is Nigg et al. 2024.
  • A secondary source attributes an ADHD alexithymia prevalence of "34–70%" to Shaw et al. 2014; do not use this figure.
  • All developmental screen findings are associational unless marked RCT. Press coverage overstates causality.
